Group Coaching Recording Waiver

Joy in Family Medicine Coaching Services (the “Company”) creates and hosts group coaching calls, often referred to as Renewal Rounds or Group Coaching Calls. I agree I have agreed to appear on the recorded group session as follows as a condition of appearing:

I consent to the audio and video recording further agree that the Company shall be the exclusive owner of all rights, title, and interest in the recordings of the coaching (the "Coaching Recordings"). To the extent I might otherwise have any interest in the Interview Recordings.

Other than transferring ownership of the Coaching Recordings, this Guest Appearance Waiver and Release does not result in the transfer of ownership of intellectual property.

I agree that I will not receive any monetary compensation for my appearance on the Video.

The Company respects the privacy of its customers and will not disclose any information I provide except as set forth in this Agreement. 

As a condition of participating in the Program, I hereby agree to respect the privacy of other Program participants and to respect the Company’s confidential information.

Specifically, I shall not share any information provided by other Program participants outside of the bounds of the Program unless I receive express written permission from such other participants to share the information.

Company shall store and host the video in a responsible secure manner.

To the fullest extent permitted by applicable law, I hereby irrevocably waive all legal and equitable claims relating to or arising out of my appearance on the Video Recording and from the Company’s subsequent use of the Video.

By registering for the call, I am electronically agreeing to the terms here within.
